Downloading Instructions, please read first:
When you download the application, be sure to save it to the hard drive on your computer before you begin to edit it. When you save the file, you may name it whatever you wish.

Note:  The file is in Microsoft Word 2002 format. If your computer does not have this program, please contact the web administrator. In your e-mail, please specify what program you have.

No Microsoft Word?  Download the Application in Adobe PDF format. Click on the icon to download a free copy of the Adobe Acrobat Reader. (You will need this to download the file, if your computer does not have a copy)

• Click on the button above to download the application to your computer.
• When the file appears on your screen, select from the command bar at the top of the screen, "File" and "Save As."
• Select "My Documents" as the folder to which you will save the file. You may rename the file if you wish.
• Press the "Save" button on the screen.
• Open your "My Documents" folder and double click on the file you just saved. You will now be able to edit the application.
